Pre-Check-In:

Your check-in messages will look different.  You will get a text message with a link.  If you have text messaging turned off – please call us to change this option.  It is the only way you will be able to complete your paperwork BEFORE you get to the office.  IF you do NOT complete before your appointment, you will need to show up 15” before your appointment time to complete the Pre-Check-In.

The Pre-Check-In link will look something like this:

By completing this Pre-Check In – it will lessen time in the office.  This link will be sent to the preferred phone number.  Please complete as soon as you get the link (a few days before your appointment.

Is paper still an option?

No.  ALL physician visits (Well, Sick, Injuries, Follow-ups, etc) will require use of the Pre-Check-In link.  If completed before you arrive, it will not hold up the check in upon arrival.

What if I forgot to complete the Pre-Check-In?

No problem.  You can complete it once you arrive.  We can also send it to a different phone number if the parent who arrives does not have the preferred phone number.  Please bear in mind, this will result in a longer amount of time before you can be checked in.

What if someone else brings the child?

Please remember that you must have a consent on file for a person who does not have the right to legally consent for care (found on our web site under forms – Healthcare Parental Proxy Form)

What if I don’t have a device with me?

As long as multiple people do not forget devices, we will have an iPad on each floor to complete electronic forms.  Again, it is important to understand that the time in the office will be longer as a result.

What are the benefits of doing this paperless?

This change will improve care you receive in the office.  By completing this when you receive the link, we will have more time to review the information provided, ensure that we have your current insurance information, current personal information and understand your concerns more fully.   This will save about one 60 foot tree per year (80,000 pages).
